Common VA Forms and Terminology
Form 22-1990: This form is to be completed the first time you apply for education benefits
Form 22-1995: This form is to be completed when: 1) You are changing schools; 2) You are changing your educational, professional, vocational goal; 3) You left your program due to unsatisfactory attendance, progress, or conduct; and you’re now reentering the same program; or 4) You were receiving VA education benefits as a veteran and now wish to receive benefits while on active military duty
Certifying Official: The school official that verifies your enrollment and sends an Enrollment Certification to the VA
Enrollment Certification: This form is used by the school to let the VA know your enrollment level. Your pay is based on the information provided on this form. This is why it is extremely important that you let the Certifying Official know whenever a change is made to your class schedule. The VA calculates pay based on the start and end dates of the classes you are enrolled in if the classes do not have the same start and end dates as the term.
