2008-2009 Online Forms
In order to access the online forms you will have
to create an account. This will allow you to electronically sign the
form. If your parent’s signature is required, your parent will also
have to create an account. This will allow your parent to
electronically sign the form.
When you click on one of the online forms for the first time, you will need to create an account. You will need to use your login and password whenever you access an online form so it’s important that you remember your password. If you forgot your user name or password, or if you are having any problems with your account, please contact ngsupport@ngwebsolutions.com for assistance.
You will be able to make changes to your data before submitting the form. If you notice a mistake while you’re entering your data, you’ll have to continue with the rest of the form before you’ll be able to go back and fix any mistakes.
You will be able to print the form after you complete it.
